Benign thyroid inclusion follicles in cervical lymph nodes are not an uncommon finding that we can encounter occasionally in surgical specimens from total thyroidectomy and lymph node dissection. However, when a few follicles or a single follicle of suspicious metastatic papillary carcinoma is found in the cervical lymph nodes, it is really hard to tell those that are metastatic lesion or just benign thyroid follicles. Does anyone have a simple but comprehensive solution for this matter? Any comments on histological differences, any recommendable immunohistochemical staining (eg. Galectin-3 or other proliferative index), or any investigative work related to this are welcome.
