thank you so much for your response but again i do not know what is the Nordic questionnaire format,i mean all what are shown in the articles are the old question not showing if i should use the generic form plus low back pain form or only low back pain is enough ...and how the score will be estimated

Oswestry Low Back Pain Disability Questionnaire, This questionnaire is more about the functional disability with low back pain and I personally prefer it.

Dr. Mohammed ..Oswestry questionnaire was performed mainly for chronic low back pain and disabilities associated ..and my aim is to identify the prevalence of low back pain and risk factors associated, i think Nordic is more suitable than Oswestry...but the problem i need to see a new fashion format of this questionnaire ..
Actually it's hard to say which is actually the best one. It depends in which environment you are using it. Should the questionnaire guide management? Should it tell about the risk for chronicity? Will it be used primarily as an outcome measurement tool? Are you dealing with chronic or acute pupulation?

Wow, that is a big question as the source of the pain and the reason for the question needs to be established first.
There are issues mentioned above - eg, is it for work? ability to undertake ADL? what age-group? what does the person want to do with his/her life (eg a retired person, or a teen-ager), what are the present capabilities, ( paraplegic elite athlete or unfit 50 year-old).
Generic questionnaires can produce a set of interesting answers but they may not relate to the most important questions.
