Have you seen bologna score? it is quite popular and has only a handful of items, many of them around the areas you are interested in. It is an observation tool, and is freely accessible on google. 

I am curious to know what kind of association you are expecting between PPH and breast feeding.    The only association between PPH and breast feeding is that if you bleed enough, you get Sheehans disease

Other than Sheehans,  breast feeding is not related to PPH.  Women breast feed because they want to except for those who do not want to, who do not. 

Every cesarean involves a PPH- the woman loses a liter of blood.   And they can breast feed if they want.   I am quite curious where you are going with this.   You also might be interested in learning about how Judy's 3,4,5 third stage protocol results in no postpartum hemorrhage

I can certainly see a correlation between breastfeeding and PPH. Breastfeeding helps aid the involution of the uterus, so once the placenta site shrinks there would be reduced susceptibility to PPH. I'd be very excited to review your findings. As immediate skin to skin could produce oxytocin as well and could certainly start the involution process.  I'm not a fan of the constant rubbing and pressure performed by nurses to start the involution process. It interferes with the mother/baby bonding process. It's clearly necessary if this bonding isn't happening but if it is and allowed to flourish in peace, wouldn't we see reduced PPH? I'd like to think so. I know my clients in the past 12 years (over 200 hundred women) who have enjoyed bonding and breastfeeding immediately after birth (home or hospital) recover faster and were never at risk for PPH. More data is required, of course.

I am not expert in paediatric or obstetrics and I do not like to disrupt your thinking 

but I suggest:

First you explain which is dependent and which is independent variable? Are you studying the effect of initiation of breast feeding on the risk of PPH? or the opposite.

Second, If it is the first then start with.statement of hypothesis:

Women who immediately initiate breast feeding of their babies are at lower risk of PPH

Third prepare a list of variable to be included in your questionnaire These may include:

variables related to pregnancy characteristics

Variables related to type of delivery

Variables related to past obstetric history

Variables related to any bleeding tendency of the mother

Variables related to the baby 

Variables related to particulars of breast feeding, timing, frequency  ...etc

Fourth: The outcome which is the PPH 

Once you make a sufficient list of variables you try then to prepare your questions and questionnaire. I can not advice you on any known questionnaire because  my research experience  is not on these issues.

What is your aim ? PPH is an emergency,breast feeding is not.PPH should not be taken lightly.Question of breast feeding would arise AFTER third stage of labour and in absence of PPH !!
