One of my close relative has adrenal adenoma on one side and kidney disease also. Will unilaterlal adrenalectomy improve kidney functions or worsens kidney functions ?
it's not related, unless the adenoma is functioning, so the surgery will improve the control of Blood pressure, and this will prevent the hypertension damage.
I agree with Dr. Azzawi there is no relation between adrenalectomy and renal function.
But as the patient have renal disease the surgery should be done with precaution as surgery may worsen renal function (hypotension and nephrtoxic may induced further renal damage) so the patient need:
preoperative nephrologist consultation to optimized renal function before surgery.
Good perioperaive fluid management to prevent hypotension or fluid overload.
What about eGFR, will it decrease further after surgery? if yes, will that be temporarily? and can come back to the same value, for example the current eGFR is at very low level already at 35 (normal value is more than 60).
Thanks dear subhash without above precaution the GFR will decrease due to perioperaive effect of anaesthesia and hypotension. So I strongly recommend above precautions to prevent further deterioration in renal function and sometimes the nephrologist suggests preoperative session of renal dialysis before surgey to optimized renal function. Sometimes the decrease in GFR temporary but it can permanent if above precautions not consider or unexpected event during surgery happened (e.g sever unexpected bleeding occur during surgery which lead to sever hypotension induce further damage in renal function )
It is not related if kidney function is ok,but if eGFR is low you have to consult nephrology before surgery and need dialysis preoperation because electrolytes disorder happen in low GFR in adernal surgery
After adrenalectomy, there was a hugh imbalance of electrolytes, Sodium went very low and potassium went very high, It took a while When my endocrinologist and nephrologist worked together between lasix and florinef medicines and now after 3/4 months, they are under control. Egfr seems improved from 20 to 30. Thanks to everyone for sharing their expertise.
