From the UniProt entry: http://www.uniprot.org/uniprot/ you can add or remove columns from the main table (the table contains 84501503 proteins on 9/10/2014). Once you have the three fields (columns) of information that you're after for every protein, click the download button and select the format you want. You will probably want tab-delimited as the default .xls format will have problems opening in MS Excel. Alternatively, you could get the whole database from the FTP site and filter it in R or similar for the columns you're after. Good luck!
