My sample is blood plasma.. If i need to do the GC-MS analysis to check the metabolites, is it necessary to add internal standards? Or can we do sample preparation without that??..

I strongly recommend to use at least one internal standard, it can solve many problems down the roads.
Yes, if you want to do quantitative analysis. However, for qualitative analysis, you don’t need to.

You have to understood the concept of Internal standard uses in chromatography. Generally, Internal standards is use where sample variability. Many times during GC samples preparation observed high variability like sample is to be extracted and processing through GC.
In-short if you find more variability then you can used. If you find no variability then used method without Internal Standard.
