A 21 yr old Primigravida 16 wks gestation married 1.5 years, came with heavy bleeding for 1 day at home. USG done a day before showed macerated fetus of 15 weeks. She also had another scan of 11 weeks 1.5 mths back. She is clinically stable with Hb - 9.4gm%. On admission to our hospital she had bleeding through the UGS which does not even admit tip of a little finger. Small clots were seen coming out through that. She passes urine through same opening. USG done at our hospital showed an empty uterus and RPOC's in endocervix and vagina (2 cm and 3 cm communicating) without any blood supply. It is the placenta.
There is a blind vaginal pouch below this which can be depressed in the site of the vagina for about 6 cm. I tried to evacuate these POC's by pressing through P/R but it is not getting evacuated. She was left for spontaneous expulsion but today's USG is exactly the same as 5 days before. How do I evacuate it? Maybe under anaesthesia I should dilate UGS a little and put ovum forcep through that and evacuate?
How did she become pregnant? We asked her and her husband about sexual history. Neither of them had any complaints with intercourse. She is menstruating normally and has no complaints. She has no urinary complaints. She became pregnant without correction of UGS. Do I need to correct the UGS at a later date? My answer is Yes. Why?
