I have submitted a manuscript to a reputed journal. After 2 months of peer review process, the response was “moderate revision has been requested” and they told me that the new version is required within 1 month.
After making the necessary adjustments, I have resubmitted the revised manuscript back on the 14th of October 2017. On the 21st of October the status changed from “with editor” to “under review”. To date (16th December) the status is “under review”.
In fact, I had made some readings about late reviews, I found that reviews are normally due 14 days after the invitation is accepted by the reviewer.
Should I worry about manuscript rejection?

No it happened with me ! my revised article was accepted after one year! they take too much time ! but at least they appear immediately on pubmed! some of our articles show only on google scholar not pubmed
I don't think you should worry about the rejection of your paper. Indeed as Subir indicated, if you have responded to all reviewer comments and have revised the manuscript accordingly, in principle the editor will take this into account. Furthermore, the step from moderate revision to rejection would be very questionable and the step towards acceptance is more logical.
The speed of the review process depends very much on the combination of journal, (associate) editors and reviewers. Therefore, it is very difficult to say something about Elsevier journals - they publish thousands of journals. Since the speed of the review process partly determines the reputation of journals, nowadays journals are more aware of this part of the review process and some of them even mention the average time it takes for different steps in the review process.
Anyway, I would recommend you to send an email to the editor kindly asking about the status of your paper.

No need to worry yourself about rejection of your manuscript in as much as you complied with, and effected the corrections observed by reviewers. I observed recently, that delayed, poor, and ineffective communication /feedback from some Journals' editors/reviewers are contributory factors to the feeling of manuscripts rejection by authors. It is equally unethical and sad that, some editors do not communicate the outcome of manuscripts 2,3,4,5, even 10 months after it has been reviewed and rejected.
I agree with previous comments and the only thing that I can add is my own experience. Some reviewers are better (more constructive) than others. The ones that are more prompt than others are sometimes but not always the better reviewers (the better ones might have a busy schedule). If your previous reviews were constructive, then that is a good sign. Good reviewers will eventually do the right thing, but the busy ones might take some time to do it (sometimes the editor has to remind them of the schedule). So don't worry, just be patient, if all of your reviewers were constructive. If you had one or more bad reviewers (not constructive) then you still don't have to worry because your paper has already made it far enough in the review process so that the burden of proof is now on the reviewer to convince the editor to reject your paper.

It depends on whether you are able to either comply with the revision, or convincingly argue that it isn't needed.
I agree with Dr. Edmonds.
If the editor is satisfied with your replies, I think he will accept the manuscript. I have got an acceptance for one of my manuscripts after two major revisions. So, be optimistic!

In my humble opinion, three factors affect the manuscript acceptance. They can be summarized in the following questions:
1- Are your responses to reviewer's notes convincing?
2- Is the editor willing to give you other chances in order to make necessary adjustments?
3- Is your research work novel and deserves to be shared with others?
If the answer is "yes" for the three questions then your manuscript is most likely will be accepted.
Hassan, the ‘under review‘ status requires a lot of patience in the journal publishing ‘world’, because there’s usually no feed back time frame provided:
First, it takes forever to recieve feedback from the editors,
second, when the feedback is finally given, for most cases the author is either told to edit for publication or simply non publishable
however since your manuscript has recieved a positive response, it’s an assurance that it shall be published, continue waiting patiently as reviewers may have a backlog in their work.

Bassam Bou-Fakhreddine, that is a good question. I don't think there is one standard procedure and it depends on the individual journal. That being said, there are things you can do to ensure that you are giving yourself the best chance of having your article accepted. Check out this blog with the 'ABC's' of responding to reviewers.

I am waiting for the response of my resubmitted manuscript. It is under review since March 15th, 2019. However, I figured out that there is a "Revision due date: May, 4th, 2019". What does that mean? What happens if the reviewers do not meet the review? I think that is an unusual case. I hope the editor may accept my manuscript anyway.

The revision due date is probably meant for you as a deadline for the submission of your revised manuscript. Reviewers also are provided with a deadline, but exceeding the deadline will generally have no consequences and will only delay the review process. If the recommendation for your initial submission was minor or major revision and you have carefully addressed all reviewer comments, there is a good chance that your manuscript will finally be accepted.

The best course is to wait for the editor decision. Things may take longer than expected.
Yo don't have to worry my friend, the article will soon receive an editor's decision which either is reject or accept. In your case, acceptance has higher chances being that the first decision was moderate. However, if your resubmitted draft is inconsistent with the initial, then the editor will have no option but to reject. Such inconsistencies often cause the delays as the editor struggles to clear the air of any suspicion.
Everyone should practice patience in this process. Each journal is different in the manner (and amount of time) they take to consider each paper.

and only to have it in consideration for similar cases, how long was your paper under review after resubmitted? My revised manuscript (after minor revision) is still under review for more than 3 months and I have not any response yet.

I submitted my manuscript in Elsevier in may last week. I got minor comment by mid of July and then i resubmitted to the journal in the end of july 2019. Till now there has been any update.

It will sent to the same reviewers to confirm there comments have been addressed or no, then they give the final decision. Accordingly the editor will give his final decision.
Follow steps carefully and revise accordingly within time period.
When you resubmit your revised manuscript, both the editor & reviewers will check your replies to their comments within specified time according to journal obligations. After that, if the reviewer accepts your revision, he/she will recommend your manuscript for publication by submitting his comment to editor confidentially while when he didn't accept your revision he may ask for more revisions or submit negative feedback to the editor.
The final decision remains to the editor himself if he feels your paper will interesting to his journal's rereads.

hi, I have been got back my manuscript from a renowned journal after about 2 years from the submitted date with major and minor revisions that suggested by 2 reviewers and have given me to resubmit by 9 months' target. After that, I solved their raised almost all questions and resubmitted by 7th months. the 2nd submission already has passed one year and still, it is showing reviewer assigned (between these times twice have changed the date, but the status is still the same), In total it has passed more than 3 years and yet without the final outcome. I'm really afraid now because I didn't face such an experience before. Is there anybody who can share a similar experience or can say what is happening to my resubmitted manuscript? does it still have a chance to get a rejection?

Bassam, unavoidably sometimes reviewers get behind schedule. If too much time passes, the contact the editorial assistant or the editors to get info about the status of your paper. I was in such a limbo for 5 months after a resubmission, but then everything went ok and got my paper published.
Now that I am reviewer for several journals, I understand that too much workload can cause delays... editors are not happy about that but this is just the way it is...

I think in case of minor revision, the manuscript is reviewed by the editor only in order to give the final decision ....that is why "editor invited".
Moderate revision is good sign for acceptance. Follow instruction carefully and communicate earliest.
It will be sent again to the reviewer(s) and most probably they will accept, then editor will accept accordingly. The next step is proof checking, then online publication
I never got a moderate revision. Usually it is major or minor revision. I guess moderate is some where between the two. If you responded to the questions raised it will be 99% accepted without further review.

Like Michele, I have never gotten a moderate revision as an author. All have been either optional, minor, or major. But as a reviewer I think I remember an option where a revision is called mandatory but the reviewer trusts the author to do what is requested so the reviewer will not review the paper again (but the editor will check the next revision). If this is the meaning of moderate revision, then the paper is sure to be accepted if the editor is satisfied with it.
