I am working on a research proposal and I need help determining the statistical model and analysis. The data collection method is survey, responses are in frequency: Always, sometime, never.

you seem to have ordered categorical data. You can use whole range of methods available for this. But in brief, you can use proportions, odds ratios, confidence intervals, logistic regression etc.
Descriptive studies try to describe findings and not to fit models, so I don't really understand the question. Tabulate the data, show plots where possible.
You may use categorical based methods. You can also do a frequency analysis over your data. I suggest you to look into "data analysis" add-in in Excel or for more advance methods, check "Analysis"--> "Descriptive statistics" section in SPSS software.
You can use log-linear models and also correspondance analysis like techniques. pl see the following reference to R package for the same:
