Well, that depends on the countrie's availability of drugs.
However, there is a recent Systematic Review of Cochrane (2014) which I consider very useful and updated about this (attached).

Dear Figueiredo In Sudan we still believe in Benzyl Penzoate 25% solution in adult and half concentration in children above 2years, daily after bath for 5 days allover body specially flexures and webs with exception to the face.
In Colombia oral ivermectin (200-250 ug/kg) or topical permethrin 5% are commonly used for sarcoptosis.
In Iran, we don't use lindane because of the high resistance of the Sarcoptes Scabiei to this agent. We use 7-12% precipitated sulfur in vaseline as a base for three subsequent days.
in my opinion, the best treatment for Sarcoptes scabii is the Benzyl Penzoate 25% and sulpher mixed with vaseline to make a dermal cream
In Portugal we have also Benzyl Benzoate 25%. However the problem is that Sarcoptes Scabiei hominis is already resistant to this medication and, as I have said above We don´t have lindane or malathion. So we need to use precipitated sulfur at about 8%.
Amer aziz do the same thing, because probably what works is the sulfur and not the Benzyl Benzoate.
The question from my good friend and outstanding dermatologist Americo Figuereido is relevant due to the variable incidence of scabies in different countries as well as the price and availability of the diverse active therapies in different countries. In Spain my preferred agents are topical permetrin and oral ivermectin, depending of the individual characteristic of each patient (young children, elderly ones, immunocompromised, etc). In fact there are no commercial presentations of ivermectin in my country, but it is possible to prescribe individually as oral formulation. Probably my answer will not help my geographically distant colleagues but I believe it is always useful to communicate by this way.
