Successful cardiac defibrillation means that the previously abnormal rhythm has been now reverted to sinus rhythm. Most important physiological factor in this would be the time lost before attempting defibrillation. So, while performing CPR, as soon as defibrillator arrives, it has to be applied, not waiting for the 2 minute CPR cycles to complete. The important thing is however, not to interrupt CPR for more than 10 seconds. Not all defibrillation attempts result in sinus rhythm, some may end up with asystole. Time factor and continuous CPR are the most crucial things.
However, If you are not successful with your attempt to defibrillate, which means that the heart has now gone into asystole, you must check the H's and T's. Hypoxia, Hypothermia, Hydrogen ion (acidosis), Hyper or Hypokalemia, Hypoglycemia. Thrombosis (coronary), Thrombosis (pulmonary artery), Tension pneumothorax, Trauma, Toxins, Tamponade.
