Actinomyces may be cultured on Blood agar (BA) with sheep blood. Of course these microbes are anaerobic (A. israeli, ), so keep that in mind. Under the first link you'll find an article how it may look on BA.
These papers are on media to culture Streptomyces species, although they may be useful also for Actinomyces. I've read somewhere that for Actinomyces you may use YEME agar without glucose. Concluding from their habitats (e.g., mouth) rich media should do.
You may use 5% sheep blood and chocolate agars, CNA, and HBT incubated at 35° to 37°C in 5% to 10% carbon dioxide (CO2) within 48 hours of inoculation. Keep in mind that although most Actinomyces spp. are facultative anaerobes, some relevant species are strictly anaerobic, so cultures must be incubated in an anaerobic atmosphere.
You can also use semi-selective media such as mupirocin-metronidazole blood agar to increase isolation rates. It can inhibit overgrowth of concomitant organisms. Other enriched media that can be used for Actinomyces isolation also includes Brucella Blood Agar with hemin and vitamin K1 and brain heart infusion broth.
