I am a bit confused about the primer design. If the same gene is expressed in different tissues, why should the primers be designed in a tissue-specific way instead of using the same primers to the same gene regardless of in which tissue it is expressed?

If you talk about some mutations (eg. Tissue-Specific Expression of a Splicing Mutation) with respect to some diseases then the haplotypes may differ in having or not having the specific exon after sequencing so that could be the reason for tissue specific expression of designing primers.
There are other cases like methylation-specific PCR (MSP) assay.
Otherwise I do not think so. Most of the Primers are universal that are used in genetic diversity studies. Do you not come across mitochondrial primers etc. Let it be any gene sequence is similar. When you design primers there may be difference at one or two bases that is not at all a problem to anchor, you get your product.

It depends, if the gene that you are trying to analyze has tissue specific variants, then you will need to check if your primers will amplify the variant expressed on that tissue. If your primers were designed in order to amplify an exonic region found in all the known variants, then you could use the same set of primers but you won´t be able to know the proportions in which the variants are expressed.
