But what about the non-genetic causes of, say, Parkinson's disease - the same applying to other symptom-defined neurodegenerative diseases as well. Why not first eliminate the vascular (arterial, venous, arteriovenous) affections which have been shown to cause NDGs as well?
You might check via my addendum how much still waits to be interventionally done in extra-genetic research domains.
I agree but as the title of the article states, the focus was on precision medicine. Unfortunately, eliminating vascular complications, which may in part be due to lifestyle, is difficult at the very least. Having says that, that doesn’t mean that we shouldn’t try. Thanks for the comment.
Thank you for heeding the - really difficult, at the very least, because of their ambiguous morphological nature - ways of specifying diseases simply on the basis of clinical symptoms.
Needn't the morbid conditions being focused on first to be unmistakably and indubitably set apart, and this in the domain of sight? Not just on the basis of clinical phenomena remaining tentatively explained in terms of conventional assumptions?
Not being sharply focused on the concretely perceptible, can we expect to put things in the proper context? Will relying on statistical probabilities allow us to spot indubitable cause-effect relationship?
Cause and effect is often hard to determine. Case in point- environmental risk factors for NDDs. Even our current testing methods will improve in time and may discover previously unidentified genes that may be associated with the development of NDD. Great example is pharmacogenomics. Our testing methods are still evolving to test genetic variants that may affect drug metabolism.
Why not look twice? See what's found out to initiate the degeneration, atrophy, down to inflammation, preceding "prelesional" changes observed with any not really informative histological trivialities, and as little informative sensory, motor, mental, etc. neuropsychiatric disturbances or deficits?
Why miss out on what longitudinal arterial, better differentiated microvascular, serial venous studies are apt to reveal? in exploiting the amazing advances made in MR imaging? their potential to identify differing structural substrates underlying one and the same NDD? allowing to watch or trace specific ways of their being brought about?
Pharmacogenomics, hadn't it better rely on unmistakably identified physical conditions? Why allow oneself to be blinded in the latter respects? to be led, first pathogenetically, and further etiologically, astray?

Is medical precision just a matter of genes, metabolomics, and drugs? Isn't the notion of (principally unexplainable) neurodegenerative diseases somewhat simplistic? More and more pathological processes presenting with the same neurological manifestations were already shown and may further be seen to escape the commonly available clinical diagnostic routine?
