A CT X-ray generates voluminous data in a 2D plane and re-constructs the imaging in 3D for visualisation purposes to know about internals. Though it was very popular in medical imaging earlier, it is now becoming more popular in product inspection, re-engineering etc.
3D slicer is a good tool to convert raw image data from CT or MR imaging equipment into CAD files. It's also open source and you can download it from their website: http://www.slicer.org/
Unfortunately I'm unfamiliar with PACS and how it could adversely effect 3D slicer with image files but I imagine it shouldn't.

Although not open source, Materialise Mimics is a reliable software. http://biomedical.materialise.com/mimics. It allows you to import DICOM data and create 3d models
I agree with Heather, Materialise Mimics is probably the best option, they also have different software packages more oriented towards specific fields, such as dental implants.
Also interesting is Amira, great results can be obtained also with it, such as exportation to CAD and prototyping files (typically .stl as also does Mimics) and FEM based analyses.

Both the people from Mimics and from Amira provide trial versions. For open source 3D viewing of PACS and related Dicom files, you can try some Matlab-based procedures available at the user community from Matlab.

Mimics and Simpleware might be the best out there for both, industry and academia.
However in my experience, Invesalius gives amazing results for similar applications, it's very stable and its opensource.

3D slicer is awesome not only that its free of cost, but in this software saving file options are very less (only .stl ....etc), if it had IGES, its seems very high compare to another
