I want to cloned my gene in pCS2 vector and then express in mammalian cells for protein expression. So, do I need to add Kozak sequence with my primer before ATG?. While, my vector already contains SV40 Poly A signal at C terminal, so do I still need Kozak sequence?

It sounds like you need the Kozak sequence. In general, it is necessary to include a Kozak sequence as it initiates the right start codon. Other sequences in the plasmid, such as the SV40 promotor/polyA signals will enhance translation from ATG without Kozak, however it is advised to include it.
