I have seen it related to superior limbic remanent sutures after extracapsular cataract surgery or glaucoma filtration procedures, Also, in patients with deficient corneal epithelial adherence syndrome like in recurrent corneal erosion syndrome, as well as toxic medicamentosa keratitis.
