40 year old patient with no risk factors for toxoplasmosis. Immunocompetent. Fever with cervical nodes since 2 weeks. His toxo IgG and IgM both are positive. HIV negative. Started on toxoplasmosis treatment in view of acute toxoplasmosis. Is there any indication to repeat his serology at any point of time? If so, then why?
Kindly provide suitable references. Thnx

In a general way, a laboratory test can be "false positive" when it is positive in absence of disease. On the contrary, a test is "false negative" when it is negative in the presence of disease. In addition, it is unlikely that both classes of antibodies against Toxoplasma (IgG and IgM) could be contemporary "falsely" positive. Finally, since in the case presented by Dr Preet there is a picture described as "clinically compatible", there are no reasons to suspect a serological "false positivity" and a confirmation by PCR is just a waste of resources. I agree with Dr Adam on the uselessness of repetition. In fact, the increase of IgG titre as well as the shift IgM>>>IgG have no correlation with the clinical evolution and have no diagnostic value.
Toxo IgG avidity may help distinguish true from false IgM+/IgG+. Though unlikely, it is possible that positivity for both IgM and IgG is actually a false positive.
