This interesting paper shows how you can use an SLM to produce Hermite-Gaussian and Laguerre-Gaussian beams.  Perhaps a similar approach might work for Bessel:

1. Yes you  can definitely use an axicon lens to produce a zero order Bessel beam. You will get a zero order Bessel beam when using a Gaussian beam together with your axicon. Higher order Bessel beams require the use a a spiral phase plate together with the axicon, essentially passing a vortex beam through your axicon.

2. The apex angle does not technically determine whether or not you will produce a zero order Bessel beam, but rather it defines the properties of your Bessel beam. The depth of focus (DOF) of the resulting Bessel beam is dependent on the apex angle/alpha angle of the axicon. The Edmund Optics link that Alan posted shows you the equation that relates this property.

3. You can make Bessel beams using different methods. As Alan mentioned previously, Spatial Light Modulators (SLM's) are used to do this. In fact, my research group does this on a regular basis with an LCOS-SLM. The other way that you can produce a Bessel beam is by using an annular slit and placing that in the back focal plane of a lens. This is described nicely by McGloin and Dholakia in the attached link. However there are some limitations with this method depending on your applications.

The thing you mentioned zeroth-order indicates the highest intensity in the central core. An axicon is an element that can produce a zeroth-order Bessel beam, whereas the apex angle defines the central core width of the generated beam. You can also consider the Fourier transform of a Bessel beam which is a ring as an aperture, and illuminating the ring along with a convex lens will produce the same beam, but in this case you are limited by delivered energy in you beam and may obtain oscillation in the on-axis profile.
