I am working on my master's thesis using an existing dataset. I would like to include the PCS and MCS from the SF-12, but I want to confirm that the way these scores were calculated is correct. Are all 12 items used to calculate the PCS and MCS? The directions I found weigh each item differently, with different weights for each component score, and that the sum of the items is added/subtracted from a national norm (or something like that). I want to confirm this is the standard way of generating the PCS and MCS from the SF-12. Thanks in advance for the help!
If you are using Stata, Phil Ryan has written an excellent command (issue the command "findit sf36") that scores the SF-36, including variant versions used in US,UK and Australia. It also carries out range checking, creation of value labels for items, and some other useful tasks that I cannot now remember.
The SF-12 has a different scoring protocol to the SF-36. Also, I think there are differences in scoring for SF-12 and SF-12v2 so you need to ensure that you are scoring the right version. There are a few websites that score both the SF-36v2 and the SF-12v2 online. This is a good way of checking that the calculations you have done are correct.
Quality Metrics, the owner of the SF12, no longer provides the scoring algorithm in order to foster the use of their scoring software for accuracy. We have used both their scoring software and our algorithms for calculating subscale and scale scores; you describe the correct the correct approach, and the older manuals clearly describe the many required steps. You are correct that all 12 items in the SF12 are used for the MCS and PCS. Some folks (and I am one) recommend using only the MCS and PCS from the SF12, and not the subscales, because the subscale reliabilities are lower (and not really acceptable) compared to the corresponding subscales in the SF36 (which has 3 times more items !).

We have patients with vestibular schwannomas fill out the SF-12 every clinic visit and we now want to analyze some of the changes before and after treatment. I understand that it has to be compared to age-based norms for Canada/US population but I do not even know how to change a ' all of the time, most of the time etc" type question to a numerical form. I came across an Microsoft Excel program for assessment of SF12 scoring. It gives both PCS, MCS and Total Scores. Pls find the attachment. I tried the spreadsheet myself and it is working very well.

I was wondering whether the the excell spredsheet is reliable for PCS MCS and Total Scores? You get a score from 0 to 100 for each of them but it seems too easy not to use the algorithm calculation...
Are these calculations are equivalent/similar and I can replace the algorithm by the spredsheet?

@Rejoice Mashaba, the online calculator doesn't save automatically but you can print out the scores. After you fill in the questions, the calculator will display the PCS and MCS scores for you to write down or print the report.

The spreadsheet is far too simplified and completely inaccurate. If you check the spreadsheet vs. the website link above you will find massive differences.

Page 21 explains how to score and Page 25 shows the weightings and complexity that is missing from the spreadsheet.
In reality, if you want to use the SF12 you need to have the appropriate license and use the actual formulas otherwise the validation of the questionnaire is invalid.

There are 2 versions of the SF-12. V1 is easy to score, they have it above, I think. V2 is normed using population survey data and is extremely difficult to hand score. You have to weight the question based on the normed data then apply a pretty complex csoring algorythem. I was able to make it work using STATA (search google for code using SAS or SPSS to score the SF12 V2). it will be really challenging using excel. GL!
