I have read repeatedly - and lately more frequently - about two related serious ethical problems in publishing: that on a published paper (a) some REAL authors have been removed from the list of authors late in the publication process, and (b) that some researchers that have not been involved in the writing of the paper at all have been included as authors. 

This strikes me as highly unethical. Have you heard about this? And have YOU become the victim of such an unethical conduct? Let us all know about such cases! How can scientific journals cope with the problem? (Do they care?) I have read about cases where authors are required to state their individual contributions, but even such statements can be faked, of course.
