The SAR-CoV-2 virus, causing the COVID-19 pandemic, is an emerging virus for which we do not yet have a specific antiviral treatment to limit or control it, and I am currently vaccinating to prevent its infection in humans. Little by little we are learning its total biochemical, genetic and molecular structure, in order to find effective antivirals and vaccines.
The pandemic as such can have several annual or biannual cycles if epidemiological measures are not taken to limit and mitigate it, such as social isolation (quarantine) use of copper mouths and protective glasses (gobles or face shields) for when people have to be exposed to a probably contaminated environment, either because they have to work, or go out to do various face-to-face procedures or buy food for subsistence.
Spontaneously, there is likely to be a limitation, with a high cost in lives (lethality) and everything related to the death of specific people, such as their social, work and family role; among many others.
During the pandemic, several open clinical studies and case reports of patients infected with COVID-19 have been carried out, diagnosed by identification of the virus in the respiratory tract by PCR and also in a complementary way by the positivity and level of specific neutralizing antibodies (AcN) in blood.
In these cases, after studying the contacts that could also be infected (family, coworkers, school; others) the following was found:
Of all of them, those of groups 2 and 4 stand out, who undoubtedly have the virus and also infection with little or no clinical manifestations; but that if they were contaminated and are carriers that can transmit the infection to other people.
It is in this group of people where some studies have been carried out to identify if there is or was an immune response; and so far, clinical and laboratory data (tissue culture and cells of the respiratory system) and experimental models in animals (rats and primates), indicate that there is both humoral (antibodies) and cellular responses (T lymphocytes and macrophages).
Conclusion:
- If there can be people with infection that recover spontaneously, only with the response of their innate, humoral and cellular immunity.
- More open clinical trials and randomized controlled and blinded trials will have to be conducted to delve into the cellular and humoral immune response to clinical and subclinical infection.
- The spontaneous response of these patients is unlikely to achieve population or "herd" immunity by itself. What if it will be feasible when added to this response, which is achieved with a vaccination (coverage) of 70-80% of the population of a country, with an effective vaccine (80-100%).

Every day we realize that scientific knowledge in relation to the covid-19 pandemic is increasing; Many of them clarify physiopathogenic mechanisms and others, biochemical or molecular findings in the structure of the virus, which guides the design of new preventive and specific therapeutic interventions (vaccines) with antivirals.
In patients recovered from severe COVID-19 infection, it has been investigated whether immunity plays a relevant role; and the results indicate that this is indeed the case, as I describe and comment on the reasons why infected patients have a spontaneous recovery.
Now research revolves around the identification of an immune profile between people who died and those who recovered, the findings are as follows:
- The virus that causes COVID-19, SARS-CoV-2, has two main proteins to which the humoral immune system responds, responsible for the production of antibodies. They are the peak protein (S) and the nucleocapsid protein (N).
- Protein N is produced at significantly higher levels in the virus than protein S, but previous studies have shown that an immune response to protein N does not provide protection against SARS-CoV-2-related coronaviruses.
- Recovered patients were found to have a humoral immune response responding primarily to protein S, while deceased individuals had a change in immunodominance such that they had a stronger immune response to protein N.
- This change in immunodominance could be detected by measuring five immune response markers: IgM and IgA1 responses to protein S and antibody-dependent complement deposition, IgM and IgA2 response to protein N.

- To date, there is limited evidence of reinfection in humans with previously documented COVID-19.
