A 13 year old child girl with osteogenesis imperfecta and severe osteoporosis and multiple compressed dorsolumbar vertebral fracture. Hx of recurrent fractures on simple trauma. What is the best treatment for this severe osteoporosis in this child?
Unfortunately, there is no specific treatment of osteogenesis imperfecta. You may try oral or intravenous biphosphonates to increase bone density. In case of long bone deformations or/and fractures, intramedullary nailing is indicated.
Dingos homages are a good options in order to decrease the number of fracture in Oi patients but not when the fracture has happened and when you operate it because the drug interfere with the healing process
Intravenous bisphosphonate therapy is the right treatment. For treatment monitoring, be guided by fracture rate, mobility, pain, bone density (DXA, if you have) and change in vertebral shape (you would hope for re-shaping during therapy). Is there a family history of OI? If not, worth exploring genetics.
Bisphosphonates (iv) therapy is commonly used for patients with Osteogenesis Imperfecta (OI). However, OI expresses itself in different range of severity and some forms of OI type ii have a high perinatal morbidity and mortality rates.
