Free but not opensource REDCap is widely used and easy to work with[php & mysql]. Open source, Openclinica is mainly for clinical trails and takes little more infrastructure to maintain[Java,oracle/postgres]. There are quite a few paid ones, If this is for managing hospital data Open MRS fits well, not for clinical studies. My experience REDCap is the way to go.
