mushrooms play a vital role in human life used for food, medicine, and chemical industries, many edible mushroom-like morels can not be grown by a human, and this type of mushrooms are called mycorrhizal fungi (myco = fungi, rhiza = roots). mycorrhizal fungi get their energy source by symbiosis with trees and certain plants.
Mycorrhizae are symbiotic fungi. Some mushrooms can be grown by human in suitable conditions.
Mycorrhizal associations is the associations of fungus and root of higher plants. Mostly the fungi belonging in this category are microscopic and can't be grown as mushroom
First, Morels are not a mycorrhiza genus - they are saprotrophs. Secondly, it is possible to create "man-made" mycorrhiza. It has been shown for Cantharellus cibarius and truffles (Tuber). But due to the difficulties and high expenses, the only species that I know of being commercialized is a truffel.

2. There are several types of mycorrhiza, one of them is Ectomycorrhiza (ECM) which can produced fruiting body outside the host.
3. Some of ECM are cultivable as Mattias pointed, but some are obligate to the host (can not be cultivated without host). But it still possible to be cultivated using it's host. However it meets some difficulties (especially high expenses and time, as many fruiting body form in long period), and we need to provide the same condition as they formed in nature. Additionally, I think the reason why some of ECM can not be cultivated without host is because it is a highly complex interaction between plant and fungi as the pose many biochemicals involved both in the symbiosis synthesis and also fruiting body formation of fungi.

An excellent answer by Ivan Permana Putra - with a complex metabolism, and need for a symbiotic association with another organism, it is not surprising that we find it difficult to provide suitable conditions for pure culture of many such fungi.

The mycorrhizal mushrooms can’t be grown by human, because they need to the plant roots to make a symbiotic realshionship with them. Thus they can’t grow without presence of these plants.
The mycorrhizal fungi provide the plant with some materials, while the plant in contrast provides the fungi with some compounds which the fungi can’t manufacture them by symbiotic relationship.
They can't be grown as mushroom, because its microscopic and needs plant roots.
