For the time being I'm using the one from E.coli 0111:B4 by Sigma. I see different kinds used by different scientists: E. coli O111:B4, O55:B5, O157:H7, J5, K12, O113; Salmonella typhimurium, Salmonella minnesota and Bordetella pertussis and... I've also seen different compounds added to the medium along with or before LPS. And some papers use the term "serum-activated LPS" for which I haven't been able to find any definition!

Depends on the nature of studies you are doing. Follow a published report and use the type of LPS they used. I' sure you are not going to compare and contrast different LPS drawn from different species of bacteria!
CD14 needs LPS-binding protein for binding to LPS. The term of ''serum activated LPS'' uses for that.
According to the product data sheet, LPS derived from E. coli O111:B4 is a specific TLR4 receptor agonist
