I work on UTI and sepsis. We find occasionally community-acquired UTI due to ESBL-producing E. coli which does not fulfill the MDR criteria (Magiorakos et al. 2013), because they are sensible to all other antibiotics. I have been searching for literature about this, but I have not been able to find studies that describe this fact.
Have you read studies that describes non-MDR ESBL-producing Enterobacteriaceae? Have you also found this?
What technique you used for testing for ESBL-production. If molecular techniques are used, your finding could be explained by the fact that gene are not necessarily expressed (they possess the gene but it is for some reasons is not expressed, thus phenotype is susceptible to beta lactams
I am a clinical researcher, we work with classic disc testing, thus phenotype is what we manage in the studies. They are just a few community-acquired E. coli that produces ESBL, without other resistance. We supose that this occurs because of a community transmission of ESBL between E. coli without antibiotic exposure, and I wanted to know more about the issue.
