What is the prevalence of an ischemic CVA following an ERCP to remove a gallstone from the common bile duct by endoscope, and is there an effect on the heart.
The cardiovascular complication and pulmonary thromboembolism (PTE) rates following laparoscopic cholecystectomy and ERCP are low, even in elderly patients (
This is an intriguing but sad case. The patient was 84 years old and deaf. The ERCP to remove the gallstone from the common bile duct was completed successfully, although did take an extended length of time. There was no bleeding from the procedure. However, there is a twist. It seems that the patient didn't hear and didn't understand the instructions properly for stopping and restarting her anticoagulant medication and did not ask for clarification. She stopped her medication a week before the procedure, but did not restart following the procedure, resulting in an ischaemic stroke, and ten days later a hemorrhagic stroke, and died two days later. The patient remained unconscious from the point of the first stroke.
Ildefonso Gustavo Díaz Sandoval Definitely. This patient was advised, but no-one checked to make sure she understood, and this patient was too proud to admit that she didn't hear what she was told. This is where I think brief written instructions would make all the difference.
Glynis Freeman Absolutely. That is why checking or verifying understanding is so important in medicine, and it's part of rapport in communication. Warmest regards
