As a clinician, sadly, I will say none. It is just like a major chromosome anomaly like trisomies that nothing we can really offer. There will be so many angiofibroma that you need to take care of, if you intend to, and more will come up. Maybe some chemicals may help but TSC is so widely involved in multiple signaling pathways and will be difficult to predict what will follow aftre the treatment.

If it comes to esthetically problematic angiofibromas of facial skin, ablative techniques are suitable, like Erb-YAG laser or simple shaving excision. If you do excisions histopathology is a must. But the result will be temporary only. 

I myself tried electrodissication (Hyfrecator 2000) in some lesions with satisfactory results.

Ablative carbon dioxide laser has worked effectively in the cases I have done. Yes, it is more of a cosmetic measure and temporary, which has been understood by patients before getting treated. The progression at a younger age with new lesions is slow, so the effect does seem to last (the progression of course would be variable case to case). 

I use CO2 laser.

Dye-laser is useful to reduce red colour in combination with CO2 laser. Topical rapamycin 0.4% has shown to be very useful (twice a day) in the reduction of the number and size of the lesions.

I agree with all previous comments. In spite of the fact that angiofibromas are only a minor problem in TSC, they are visible and when patients ask for some help we can remove. Patients are happy even they know that could recur. I destroyed them under local anesthesia by electrocoagulation or Erbium-CO2 laser. In fact, I agree with Dr. Adil H.H. Bashir and I do prefer EC because the procedure is shorter and has best hemostatic qualities.
