I read about circulating tumor cells and understood that cellsearch (Janssen) is the only FDA approved method that existing for capturing cancer cells and they are based on antigen-anibody binding. It is supposed t give high specificity and efficiency since antigen- antibody interactions are highly specific. But why there an active research going on in this topic while there existing an efficient method?
If Cellsearch system is not efficient enough, what are the disadvantages of that method? Kindly give a reference with your answer
I experienced CellSearch System. One of the disadvantages is that it is suitable only for cancer of epithelial origin.
Correct, the separation is namelz on EpCAM molecule ant this is epithelial not mesenchymal. Ver low separtion resulets in comparision with filtration methods.
I agree. It is a method that excludes non epithelial and "emt" (epithelial-mesenquimal-transition) cells.
Its correct that that its the only FDA approved method for capturing CTSs based on EpCAM Ab interaction, not taking into consideration (so far) the eptithelial-to- mesenchymal transition (EMT), which is for me a major flaw. An alternative method is to use RT-qPCR.
CTC with mesenchymal-like phenotype can be difficult to isolate through standard methods based on the recognition of epithelial markers as EpCAM and/or CKS[i]. Therefore, considering the limits now established of enrichment by EpCAM-based methodology[ii], other methods have been tried to advance CTC detection

This method is restricted for metastatic tumors, right. If you do not have infiltrating tumor cells , e.g. into the circulation, you will not find them. That means, if you detect CTCs you must have likely an aggressive metastatic kind of cancer, possibly to late for any curative resection or meaningful treatment including most likely secondary tumors lesions in other organs.

the most important disadvantage of CellSearch system is we can not keep our CTCs alive after test for following test or culture them for give more information about captured CTCs...
so the other methods try to solve this problem and isolate CTCs alive for culturing
It was not designed for it... you should do cell sorting via FACS Sorter as BD FACSAria™ III cell sorter.
