Angiotensin converting enzyme (ACE) analysis, like most laboratory test are method/technology dependent so literature review will give different ranges. Moreover, children seem to have higher values of ACE levels.
Anyway i feel the range is between 8 to 53uL.
To me the test is having less positive predictive value for sarcoidosis and is only one of the supportive finding making you consider sarcoidosis in your differential.
Moreover, the recent data had identified polymorpisms within ACE enzyme adding further to enzyme levels within blood. ACE enzyme has been associated with 3 genotypes including DD, DI, and III where the former category associated with highest values.
The levels in adults are around 20-70 U/L, but can be up to 112 U/L in children. These are for the common Buhlmann method but can vary from method to method.
Our experience in adults suggest that ACE levels are not different between men and women and the levels do not change with increasing adult age. It is actually unusual to find an analyte that doesn't change in the elderly (eg over 70 / 80 y/o).
