I have sent an article for publication 6 months ago. For three months it was under the "with editor" status. After that, it is with the "Under review" status again for last three months. Overall, it has taken 6 months. Does it indicates that the paper will be rejected or what could be the possible reason for delay?

It is possible that the editor is looking for the reviewers on your direction. My paper has been reviewed about 8 months. Up to now, there are nomessages.

Usually the time taken for peer review varies from journal to journal.And the under review status does not indicate that it has been rejected.
If you have waited for this long, you can wait for a bit more and hopefully you will get the result in favour.
I assume the Editor is finding it difficult to get a suitable reviewer, you may send a message to the Editor to share your concerns. Don't assume it will be rejected yet.
