Some of the larger companies are very flexible with their financing options. The cost of guaranteed up-time / repairs / software upgrades can easily exceed the purchase price. Virtually all systems come with computer equipment that must be upgraded and renewed at least once during the life time of the camera. The life cycle of such equipment and software is much shorter than that of camera's or measurement equipment.

Some companies offer camera's for a small nominal purchase prices and have their return on investment through a mandatory and expensive maintainance contract. Such approach works in some hospitals,

Some companies even offer pay-per-study or pay-per-view.

It is not unheard off, if you are considered a market driver, i.e. a university hospital, that you are offered a discount exceeding 40%.

All of this varies with national and local infrastructure or situation.

Doing your homework really pays off. Take it very seriously. Get advice from someone with decades of experience in the market and hospital environment.

Several large  yearly congresses, such as the EANM and the SNM have a trade exhibition as part of the congress. These are very efficient venues to make contacts, to discuss matters, to compare equipment. Compared to many other medical congresses with associated business activities, these are really lpaces to network, if you are serious about starting a new department.

Purchase prices do not tell the whole story.

For example, if a gamma camera can automatically perform its daily quality assurance procedures without requiring staff attending, you may save 5  to 10% of profitable camera time.

Some camera companies can also help you get you a good deal on the radiopharmaceutical price.

The bottom line is: know what procedures to do, how many you will do, have realistic ideas about the future development and about the logisitics of a department. And above all: know your ecosystem!!

Too many hospital directors were told a PET/CT scanner is good for 2500 procedures per year, but were not told what other requirements had to be fullfilled before that mythical number could be approached.

What matters is the total-cost-of-ownership and the total-cost-per-procedure under all possible scenario's. Simple purchase pricelists will certainly not do.

And when computing things such as cost-of-ownership also put figures in for the cost of down-time [there will certainly downtime] or the cost and effort required when equipment must be replaced.

Only politicians and civil servants can get away making businesscases using list prises. It got Europe in a crisis!
