The woman was found sever hypothyrodism at 13 weeks of gestational age. Her TSH was 110 mU/L, FT4 4 nmol/L, and TPOAb-, TGAb-. She was performed partial thyroidectomy for thyroid cyst at 7 year-old. This pregnancy should be continued or terminated?
Pregnant Woman with sever hypothyroidism may be on risk of abortion, and if she don't the neonate may have a hypothyroidism and neonate should receive treatment to avoid neurological disorder caused by hypothyroidism.

Dobbing J and Fisher DA showed that maternal thyroid hormones are necessary for normal maturation and myelination of neurons and glial cells, and play a critical role in brain development during the fetal period. Therefore, this patient and several doctors thougt that the brain development of this fetus may have been affected. They chose termination of this prenancy.
Start controlling the hypothyroud state by small doses of thyrixine increasin it gradually, then asses the fetus by ultrasound....not to rash to abortion.
Please find attached ATA guidelines on Thyroid and Pregncny, specially section VII. Detrimental effects of Clinical ( overt) maternal hypothyroidism on Baby are weell known. However, if fetal loss doen´t occur, treatmente with LT4 are neccesary as soon as possible. I agree with Dr. Mahgoob ...not to rash to abortion.
