60-year-old male was operated due to Klatskin tumor mistaken by benign stenosis of the bile duct. He underwent local resection of bile duct without lymphadenectomy followed by Roux-en-Y hepaticojejunostomy elsewhere. Pathology disclosed Klatskin tumor with positive margins. One month after recovery, patient received systemic chemotherapy, 5 cycles of Gemcitabine and Cisplatin. After chemotherapy and eight months after surgery he came for a second opinion. At this time MRI shows no residual disease and PET-CT was negative. What to do next? Reintervention and extended lympadenectomy and hepatectomy, observation alone (wait and see) or Radiotherapy?
