I think no standard questionnaire can be applicable to every setting. It really depends upon the target population and must be contextualized to local settings and objectives of the study.
It depends on your study population. There are some different type of questionnaires. You need to decide one of the standard questionnaires which was suggested by other friends above then you can modify it base on your needs
It is suggested to frame the questionnaire according to the objective of your topic of the study. For example, i considred Morisky scale of questionnaire for ART drug adherence assessment.
It will be important that you clarify the target population there are some for adolescents, others for adults and other for health care professionals.....So try to be more specific so we can help you.
Dear Maria, you have asked about the target population. They are diploma health science students between an age of 16 - 24 and living out of the campus.
as far as I know, that EHDS (Ethiopian Health & Demography) surveys have, on several occasions, included questions on HIV/AIDS. Since it will be quite important for you to ciompare your results to those found previously in a similar group, it would be a good idea to look there and into studies that dealt with the smae question in a comparable population. We had once done a similar survey in the Rwandean setting (health workers, and also school students; it is available here), but this is more than 10 yrs ago and things tend to change over time.
