Steroids are a most valuable adjunctive to the armamentarium available to health professionals for the management of mid and post endodontic
flare ups. Up to 80% of endodontic patients who report with preoperative pain continue to experience some level of pain following the endodontic
procedure. Since endodontic pain is often associated with chronic inflammation, the presence of bacterial by-products, influx of primed immune
cells and activation of the cytokine network and other inflammatory mediators, pain may be reduced by administration of glucocorticoid steroids.
During endodontic treatment and when managing trauma to the teeth steroids may be applied systemically (orally and/or parenterally) or locally
(i.e. intra-dentally via irrigants and medicaments).

There are many products available to administrate intra-dentally via irrigants and medicaments for this matters. I use it frequently cause generally patients comes to the clinic when pulpar status is advanced for inflammatory and/or acute infection.

The problem with steroids involves reduction in healing. If the primary reason to use reduce inflammation then short term use often proves beneficial. However, steroids impede healing and that is a necessary factor when dealing with endodontic surgery and recovery. After the initial 2-3 days application...I would then refrain from repeated doses of steroids. Additionally, NSAIDS also reduce healing so going from steroids to NSAIDS do not make sense if proper healing is necessary following surgery
