What kills is the body's reaction to the virus. The lung is rich in tissue factor, a glycoprotein produced by extravascular cells. Blood cells and platelets do not produce tissue factor. Viruses attack and disrupt the vascular endothelium that isolates blood enzymes from tissue factor in the lung. This exposes tissue factor to blood enzymes and activates the stress mechanism that continuously maintains and repairs tissues and regulates hemodynamic physiology. Tissue factor stabilizes factor VII and enables its enzymatic activity, which acts as a "trigger" for the activities of factors VIII, IX, and X. The resulting enzymatic interaction generates excessive quantities of thrombin, soluble fibrin, and insoluble fibrin, causing systemic inflammation, exaggerated blood viscosity and coagulability that invites infarction, and exudates that flood the lung and disrupt gas exchange.This stress mechanism activity is exaggerated by the presence of pre-existing disease, which is also a manifestation of stress mechanism hyperactivity. Even if the patient survives the acute virus attack, the stress mechanism hyperactivity will promote fibroblast collagen production that causes pulmonary sclerosis and other forms of tissue and organ damage, so that surviving patients may suffer permanent disability.

The stress reaction of COVID is analogous to the stress reaction of eclampsia, which is routinely treated and controlled using magnesium sulphate infusions. Why is it that only obstetricians recognize the benefits of magnesium sulphate???? It is time to tell the FDA to go to hell and employ effective treatments. The COVID victims could be quickly, safely, and efficiently salvaged using the following treatment regimen:

1. elective endotracheal intubation to support gas exchange and protect medical workers from exposure to the virus

2. Permissive hypercarbia in the range of 50-100 torr to optimize oxygen transport and delivery, and prevent lung damage.

3. Generous opioid dosage to prevent harmful nociception caused by all forms of sepsis

4. Intravenous magnesium sulphate, EDTA, or trisodium citrate to control excessive thrombin generation by the stress mechanism

5. general anesthesia 1/2 MAC to prevent fear and anxiety that harmfully exaggerates sympathetic tone and activity.

These measures will quickly and efficiently control the stress mechanism hyperactivity, restore homeostasis and organ function, and enable the body to rid itself of the virus and recover.
