In DCC coupling reaction of acid and alcohol, DCU is formed but many times DCU was not formed, the reaction did not give any product. Why?

I was doing a DCC coupling reaction of an acid and alcohol, and sometimes a product formed, but many times DCU has not formed even in addition of 1 equivalent of DMAP or HoBT. Why was this happening?
