If you do not want to implement a CBR system from the scratch you might consider the use of a development platforms such as jColibri (http://gaia.fdi.ucm.es/research/colibri/jcolibri) or MyCbr (http://www.mycbr-project.net).
From the diagnosing , perhaps you may need to transform your Health Records to a computer interpretable format. You might consider an automatic/manual markup parsi ng process (using ICD, Snomed, etc.).

Well..when I used the tool in my research, 2011-2012, only jColibri and myCBR were free. See pages 65-66 of this research (http://siaibib01.univali.br/pdf/Helton%20Machado%20Kraus%20dissertacao.pdf). It's in portuguese, ok? The author shows a comparative between some CBR tools.
I used jColibri because of the tutorial (http://gaia.fdi.ucm.es/research/colibri/colibristudio/documentation). Apparently, the Gaia group has not made any more updates since 2008.
