I have identical entries for the same book that was re-issued recently by a different publisher. How do I merge them without necessarily deleting one or the other?

You number them using A. and B. For example 23 (a). ----------- (b). ------------

It is not a must that you merge the two entries. You can present them as two separate entries. For the first one the source/s is/are the author/s and the source for the second one are both the one who is quoting the original document plus the author/s.
