I don't know how specifically this is done in Medcalc, but what you need to do is to specify first your method of combination of these two markers (the combination rule(s)). According to that rule you need to calculate combined test results (like Diseased or Not) into a new column in your dataset. Then you can calculate Sens, Spes etc by crosstabulating this new line and the results of your gold standard.
