Minimally Invasive Spine Surgery has shown to be more advantageous than open/traditional surgery on multiple occasions. Nowadays, even adult deformity surgery is being done by minimally invasive techniques. However, one of the drawbacks of this surgery remains the steep learning curve. Multiple research documents have however proven a decreased intraoperative bleeding amount, quicker ambulation of the patient and overall decreased pain! All in all, Minimally Invasive Surgery in Spine does have a bright future. However, the upcoming trend of endoscopic surgery in spine, although limited in its scope with current technology, may prove to be even more advantageous.

Minimal invasive surgery is good and its effectiveness will keep increasing with advent of newer technical advancements, instruments, imaging etc.

But sound understanding of open procedures will remain basic core skill for time. The good skill of open shall be instrumental to build better skill in minimal invasive ones.

Besides it, robotics may in future assist with even more mini invasive manner as is commonly used in cardiac surgeries eg. Da Vinci system etc..

I agree with the two answers above. MIS spine surgery definitely has bright future and will become more common and finally the standard of care for most spine pathologies in years to come. Robotic assisted surgery will decrease the learning curve.

I can tell you from my own experience as well as experience of other MIS surgeons I know that MIS spine surgery definitely causes less pain than the equivalent open procedure. There is less dissection and trauma to the body so patients have less pain, less blood loss, quicker mobilization and recovery.

Traditional open spine surgery has been shown to lead to some form of post-traumatic stress disorder due to postoperative pain in about 30% of patients. You will not see such high rates of PTSD in MIS patients.

Thank you, dr. Sinkov, for your comments. What I hear from some Czech surgeons is, that during open surgery some of the sensitive intervetebral nervs are cut and therefore the patients have less pain (!). These surgeons have tried MIS and returned back to open surgery. :-(

I would be happy to discuss the merits of postoperative pain in MIS spine surgery with those surgeons. I would hardly qualify denervation of a facet joint as a BENEFIT of open posterior spine surgery. This same open approach also denerveates and disrupts blood supply to multifidus muscle contributing to postoperative muscle dysfunction and stiffness.

MIS is advantageous in capable hands due to minimal scaring and so better results. There are risks that have to be explained to the patient.

Recent literature has demonstrated positive outcomes for patients undergoing MIS-TLIF, including minimized operative times, reduced postoperative analgesic use, decreased lengths of stay, and less overall soft tissue damage.
