You may want to take a look at, The Stigma Scale: Development of a Standardised measure of the Stigma of Mental Illness. Three questionnaires were developed, and this may be a good start for your research.

My recent article on social inclusion has a section on the stigma section of the social inclusion interview. I wouldn't regard it as a robust measure of stigma though. Look up the work of Link. He developed some questionnaires that we have used. Its important you also get data on actual experiences of being stigmatised as well as perceived stigma which the questionnaire u choose will capture.
