As the time passes, the new innovations, new technology and new concepts are coming forward. It's the natural that the changes will occur.
New researchers and scientists have discovered many techniques/technology for the welfare of mankind. It's but natural that the medical science evolving at very fast pace with new developments and old ones are like second fiddle. Changes are must for good.
With the rapidly changing world of medicine there will for sure be changes. I don't think there will be extinction of any one specialty as each have a purpose. I think think we will see new sub-specialties arise from within our specialties as well as some cross bridging of other subspecialties where overlap exists.
Molecular biology techniques such as new generation sequencing will definitely change the information concerning genetic disorders.It is hoped that eventually these techniques may contribute to the treatment of some very serious diseases.

I think it is necessary to introduce some new modern medical disciplines, but also to emphasize the importance of preventive medicine.
Priority and preference will be with the evolving medicine, but extinction of some is difficult as that might be needed in big picture.
I guess it is just matter of time before we actually see the benefits of new medical strategies to forget the old ones. Though not everything new will be worth it's portrayed label.

It always happen and many branches of contemporary Modern Medicine will be modified, some new branches will evolve and some will lose their relevance in near future.
