Assigning boundary conditions for a plate element?

I have a problem assigning boundary conditions in different nodes in plate element. When we say that two edges are simply supported and two others are clamped, how should we assign boundary conditions for the four nodes in a quadrilateral plate element?

Assume we have three degrees of freedom per node including w,theta(x),theta(y)
