More than 10 % of prescriptions are connected with serious medical errors and more than 50 % of antibiotics are prescribed in a inappropriate way. Clinical pharmacist (CP) are very important and robust tool again inappropriate prescribing. Usually each $ put in to the CP's service is pays back at least 3-5 fold. Are clinical pharmacy service included before first drug administering to the patients. Are risk assessments done for this field in your hospital? Are interventions written by clinical pharmacists to prescribers?
My opinion is that the most appropriate interventions done by clinical pharmacists are written interventions to the prescribers and feedback should be included. After that quality indicator should be established which measures this process. At the end, appropriate corrections and preventive actions should place to prevent the patients by being treated with inappropriate medications. Are prescribers in your country available for this kind of cooperation among CPs and prescribers?
