This is an old injury (8 months) which is healed. If we believe that the articular cartilage is in reasonable condition and needs osteotomy and fixation, the elevation would be less problematic if it will be open as it will be easier the placement of the necessary structural bone graft which will be used to support the bone surface and the fixation will be under direct vision. Problem with this method is that the articular cartilage of the whole joint is exposed to the air. Arthroscopical assisted surgery can be performed, will be kinder to the joint but the possibility of a fully reconstructed surface due to imperfect osteotomy and support could be a problem. In the long run she will need a kind of replacement and correction of the secondary soft tissue deformities
