Why the second generation of H1 histamine antagonist drugs such as Claritin "Loratidine" and Aerius "Desloratadine" should be administrated before sleeping although the have non-sedating effect?
It is generally accepted that modern second generation antihistamine medications do not have effect on CNS. However we get these date from RCTs. Real life studies and everyday practice show however that individual tolerance to AH differs from patient to patient. That is the reason why when I prescribe AH to a patient for the first time to give the first dose in the evening in order to test his/her individual tolerance to the medication. In case of no CNS side effects the medication can be taken at any time.
Dear professors, loratidine and desloratidine are non sedating antihistamines and even less sedating than other non sedating antihistamines like cetrizine, besides that they have a long duration of action which means that even if someone take them before going to sleep for example 10 pm their peak effect will occur at early morning which lead to more danger if the idea of using them at night to decrease risk of their sedation or to check their sedation on a patient !!!!
2nd generation antihistamine has also some drowsiness effect with some individualize differences . and also may be related to the disease pathogeneses eg more nocturnal problem and to the drug mechanism of action as the parasympathetic has the upper hand during night and so more effect and good sleep

An intake of antihistamines in high doses or for long periods seems senseless. These drugs, indeed, produce toxicity in the body and no one should lose more time than necessary with the state of mind they produce.
