In our work, the pus in the container was aspirated from renal pelvis by a surgeon in a patient from our research. The research which was done by a team work with me aimed to detect formation of microbial biofilm on double J stent catheter in ureter. During surgery, the surgeon was aspirated this huge amount of pus. i would like to show your suggestion about the type of infection either single or mixed bacterial infection and tpoe of bacteria.
I would need to know the diagnosis, I'm assuming pyelonephritis but I would need a confirmation. E. Coli, E. Faecalis, Klebsiella spp. and pseudomonas are possible with E. Coli being by far the most likely candidate. Therapy is usually a fluroquinone or a third generation cephalosporin, but given how advanced this is, I would put that person on IV Levoflaxin or ciproflaxin and maybe an aminoglycoside IM, like gentamicine, and a ressistance culture ASAP, on both the pus and the biofilm.
The presence of Serratia spp. (including S. marcescens) also cannot be excluded. These microorganisms may be the etiological factor of various urinary tract infections. The red color of pus may be associated with the production of prodigiosine, the red pigment, by these bacteria.

it is usually mixed infection, culture and sensitivity, is always highly recommended.
I just ask about the volume of asparated pus, can you tell us, because the picture shows a jar without measurements.
